It is important to recognize this variant anatomy and to identify all branches during dissection. Instead, we proceed with dissection of the right hepatic artery and portal vein, leaving division of the right hepatic duct until parenchymal transection later in the operation. The right hepatic artery is usually ligated to the right of the common hepatic duct to protect the left hepatic artery from inadvertent ligation or injury and to facilitate access to the underlying portal vein. It is important to be certain that the left hepatic artery is preserved by palpation at the umbilical fissure after temporary occlusion of the right hepatic artery. Although the right hepatic artery can be ligated medial to the bile duct, we prefer to ligate it to the right of the common hepatic duct. The artery can be divided with an endovascular stapler or alternatively be double-suture ligated to facilitate holding the proximal end with light traction to the left. This latter technique, together with traction of the cystic duct stump, is of considerable benefit in exposing the right portal vein. The portal vein is approached laterally and posteriorly and is best exposed by dividing its peritoneal covering. The main portal trunk is exposed and dissected superiorly, until the left branch is revealed and preserved. A renal pedicle clamp is passed gently around the right portal vein under direct vision. Special care should be taken not to damage the first posterior branch of the right portal vein, which comes off early and posteroinferiorly to supply the right portion of the caudate lobe. If any difficulty is encountered in encircling the right portal vein, this branch should be individually ligated and divided before any instrument is passed around the right portal trunk. It is good practice to place two retaining prolene sutures to secure the vein before dividing it to prevent retraction. These sutures should be left with the needle in place and are used to oversew the stump after the vein has been divided. During transection of the right portal vein, it is imperative to avoid injury to the left portal vein, which can be ensured by confirming flow through the left portal vein using Doppler ultrasound after the clamp or stapler is applied to the right portal vein before transection. Because it may be difficult to obtain a clear view of the right hepatic duct and its tributaries for dissection and ligation extrahepatically, these structures are best left intact and divided intrahepatically at parenchymal dissection. However, if extrahepatic control of the right hepatic duct is necessary, most often for oncologic purposes, it may be dissected out at this point. This maneuver opens the umbilical fissure and allows better exposure of the subhepatic space. Absorbable suture material should be used to prevent permanent suture acting as a nidus for future stone formation or infection. If difficulty is encountered in passing a suture around the right hepatic duct, it may be divided under direct visualization and subsequently oversewn with a 4-0 absorbable suture. Often, the ducts draining the right anterior and posterior sections are found entering the confluence separately, or the right posterior sectional duct may drain into the left hepatic duct (see Chapter 2). In such cases, both these major sectional ducts should be individually identified and secured. This approach is most useful for right-sided tumors located peripherally away from the hilus, which allows its use without compromising tumor clearance.

After infusion of Lipiodol emulsion, tumor-feeding hepatic arteries are embolized. Hepatic artery embolization induces tumoral ischemic necrosis and increases chemotherapeutic drug dwelling time in the tumor by slowing the rate of efflux from the hepatic circulation. Furthermore, ischemic damage by embolization potentiates absorption of chemotherapeutic drugs, disrupting the function of transmembrane pumps in tumor cells (Giunchedi et al, 2013). Proximal arterial occlusion is not desirable because it will not only induce development of intrahepatic and extrahepatic collateral vessels, but it will also preclude a repeat procedure. The optimal size should be small enough to reach and occlude the terminal arterioles to the tumor but be bigger than the arteriovenous shunt and peribiliary plexus, to avoid the risk of pulmonary embolization and bile duct necrosis.
